5 Simple Steps to Participate in the Creative Expression Competition
Step 1: To participate in the global Creative Expression Competition, download the global submission
form below and read the complete rules and regulations.

OR

You may participate in the Inspired by Diabetes Competition by clicking on your country's flag below.

Step 2: Create your written or visual entry.

Step 3: Complete and sign the submission form. A parent or guardian's signature is required
for all children participating under 18 years of age.

Step 4: Carefully pack the entry and submission form.

Step 5: Mail the entry prior to March 31, 2009 to the address on your submission form.
